My choice will have to be Majora from Majora's Mask.

First you fight the Mask itself which for some reason is now huge and has gross...bloody innards coming out of it....okay...

(I'm pretty sure they're just tentacles but they always looked more like innards to me)
Then he sprouts arms, legs and an eye and starts running all over the god damn place with a squeaky voice which, is hoenestly, pretty terrifying when you first see it.

Then finally he..uh...thickens(ew.)...and grows whips out of his hands to smack the ever loving day lights out of you.

All and all making for an unforgettable final boss for an unforgettable game.
