Had to think about this for a while.

Morpheus D. Duvall from Resident Evil: Dead Aim (what a surprise).

Looks like your generic japanime male villain, yes?

Well, after injecting himself with the experimental t/G-virus he becomes this:

And now I have no idea what gender he/she is anymore.

Oh, and then it goes on to this:

But let's not go into that one.

If I may add a few of my favrate:

Flea from the Chrono games

Bridget from Guilty Gear

This one from Persona 4 is hidden due to spoilers that I'm sure everyone knows by this point but I'm being save
Naoto
Ms and Mr Pacman

Let's also add

Gardevoir, who I don't think I have had a female version of.

^Good choice with Bridget!

Since me and Gamemaster have been the only entries on this one after Psycho, I'll just throw in another one of my favorites.

We will never know for sure...

Favorite Ninja Time!

Taki from Soul Calibur. (Don't Google image search her)

Aaaaaand, of course, I did. Nipple-icious. Nothing worse than what Hex put up with her figurines.

---

Old school Jago from Killer Instinct

Oooh, I love ninjas! I actually have a picture folder on my computer dedicated to all my favorite ninjas in popular culture. Currently has 92 pics in it, a few of the pictures featuring more than one ninja from a specific title. Coincidentally I have the same exact picture of Taki in there. Jago's there too.

Hnnnngh! This is such a tough choice! I can't choose between Ryu Hayabusa and Scorpion D:

That's why I'm gonna choose Ryu's favorite rival, Yaiba Kamikaze.

Where is she by the way? I haven't seen her post anything in ages.

She left after an argument. Its a damn shame too. I tried messaging her over tumblr but she didn't respond to any of them.

Can this guy count for a Ninja even though he's part Cyborg?

Oh. I remember that but I didn't think she'd actually leave because of it. I thought I had just missed all the threads she may have posted in or something. Well, that sucks... Case closed, I guess.

Ayane from Dead or Alive and Ninja Gaiden

Favorite "This isn't even my final form".

As in, any form that is neither the first or last of a character that transforms multiple times.

I'll go with Hydralisks from Starcraft 1. They start as larvae. They become hydralisks. They can then become lurkers.

My choice will have to be Majora from Majora's Mask.
First you fight the Mask itself which for some reason is now huge and has gross...bloody innards coming out of it....okay...

(I'm pretty sure they're just tentacles but they always looked more like innards to me)

Then he sprouts arms, legs and an eye and starts running all over the god damn place with a squeaky voice which, is hoenestly, pretty terrifying when you first see it.

"Hi! Welcome to Nightmares!"

Then finally he..uh...thickens(ew.)...and grows whips out of his hands to smack the ever loving day lights out of you.

All and all making for an unforgettable final boss for an unforgettable game.
I smell a loophole.... You never stated that it had to be from one game and it had to be just one person.
Look at all these forms that he has.
From left to right, we have his heartless, Young Xehanort, Master Xehanort, Terra-Xehanort, and Xemnas.
To be fair I'll also pick someone/thing from one game.

From The World Ends with You
Every boss (end of the week boss that is) has a noise form to battle with. Megumi is no diffrent. He starts out in his Reaper form battleing you along side a brainwashed Shiki.

After that you face off with his first noise form, Anguis Cantus, With the help of Shiki and Beat as your partners (2 parners for two health bars).

After that you thought you won, but he abosrbs Shiki, Joshua, and Beat to become Draco Cantus, Who has five heads and turns the battle in to a "Bullet Hell RPG"

Perfect
Majora from Majora's mask.

I was actually consider putting Xemnas too just because of the final boss fight in Kingdom Hearts 2.
