I've been thinking about it, and I'm pretty sure it's Jacob's Ladder.
It's one of those movies where you're like "WHAT THE FUCK IS GOING ON?!" throughout the entire thing and, since they use the gore/shock segments sparingly, it really gets you when they do show up.
