It's either Gauntlet Legends or Gauntlet Dark Legacy...

I'ma have to go with Dark Legacy for the HUD.

I'm not sure what the diffrance between the two are, but yes, it is Gauntlet Dark Legacy

One's cooler (Legends) and one's basically a remake of the dreamcast version.
